Request care
Request a physician visit.
Start with a text. Dr. Gildin reviews every request personally.
Is this an emergency? This service is not a substitute for emergency medical care. For severe bleeding, major trauma, difficulty breathing, loss of consciousness, or other potentially life-threatening emergencies, call 911 or seek emergency department care immediately.
How it works
From first text to follow-up.
Text the doctor
Text (201) 877-5472 with a brief description: who was hurt, roughly where and when, and where you are. A call works too.
HIPAA-compliant secure text & phone lineSend photos of the injury securely by text.
Initial screening
Dr. Gildin reviews your message and decides whether a mobile visit may be appropriate. He may ask for a photo of the wound, which you can send by text on the same secure line.
Secure forms
You’ll complete two short secure forms, a general consent and an informed consent for suturing, covering contact details, medical history, medications, allergies, tetanus history, and consent. A parent or guardian completes them for patients under 18 and must be present for the visit.
Reservation
Once your visit is accepted, you’ll receive payment instructions for the reservation and evaluation fee.
Your visit
Dr. Gildin comes to your location, examines the wound, discusses options, and provides medically indicated care.
Aftercare
You receive wound-care instructions and an itemized final payment request, with follow-up available through secure messaging.
Before your visit, complete both forms (about 3 min)
Start with a text or call (above). Once Dr. Gildin has reviewed your injury by text and asked you to, complete these two secure forms in order.
- Form 1General Consent
- Form 2Informed Consent for Suturing
